    Ingested sharp-pointed foreign bodies can cause serious complications. A case of a 16-year-old girl with cardiac tamponade due to ingestion of a sewing needle is presented. Ingested needles have often been reported as a cause of gastrointestinal injuries but in this rare case the sewing needle actually migrated into the myocardium.

    Background: Pulmonary sequestration (PS) is defined as nonfunctional tissue within lung, has separate arterial supply from lung itself and does not have any communication with the tracheobronchial tree. PS is divided into two types which are intralobar pulmonary sequestration (ILS) and extralobar pulmonary sequestration (ELS). PS is a rare congenital malformation of the lungs which is composed of 0.15% to 6.4% of all congenital lung malformation. PS is commonly located in the left lobes and its incidence, especially the incidence of ILS, in the pediatric population is higher than in the adult population. Diagnosis of intralobar pulmonary sequestration which is in the right lower lobe in the third decade of life is a rare situation.     Periosteal chondroma is a rare, benign, slowly growing cartilaginous tumors. They are found in long and tubular bones, mostly. It occurs in second and third decades of life. Rib is an rare location to found periosteal chondroma. We demonstrates a case report of periosteal chondroma founded in the rib that belongs 44-year-old man. We also examine isocitrate dehydrogenase 1 (IDH-1) mutation of this unusually located periosteal chondroma immunohistochemically and IDH-1 mutation was determined as negative.     We evaluated the clinical features of patients with flail chest, together with treatment results, and the factors affecting prognosis. The study included 34 patients (27 males, 7 females; mean age 41 years; range 15-61 years) who underwent treatment for flail chest. A retrospective analysis was made regarding the etiology, injury to the chest wall, pulmonary contusion, hemothorax and pneumothorax requiring chest tube, associated injuries, injury severity score (ISS), the presence of shock on admission, the amount of blood transfusions within the first 24 hours, treatment, and the results. The most common cause of flail chest was traffic accidents (79.4%). Shock was detected in 41.2% and pulmonary contusions in 55.9%. Ventilatory support was required in 70.6%. The mean ISS was 36; mortality occurred in 32.4%. In seven patients without associated injuries and who did not receive ventilatory support, the mean ISS was 22.8 and all survived.     Empyema is a serious complication of bacterial pneumonia in children. Between July 1992 and July 1998, 53 children aged 7 months to 12 years (mean age, 5.5 years) were treated for empyema complicating pneumonia. After diagnostic thoracentesis, closed tube drainage was carried out with appropriate antibiotic therapy and other treatment strategies such as pleural lavage, intrapleural enzymatic debridement, decortication, or pulmonary resection, according to the effectiveness of drainage and clinical status. There was one death from toxic shock. It was concluded that early decortication in the chronic stage of the disease is a safe and effective treatment modality.

    LCH, also formerly known as eosinophilic granuloma or histiocytosis X, is characterized by impairment of the structure and functions of normal tissues by abnormal proliferation of Langerhans cells. We report and discuss here isolated pulmonary Langerhans cell histiocytosis (LCH) with recent literature. Our patient is a 28-year-old male man who has been complaining of cough, shortness of breath, chest pain and weight loss for three months. Thoracic computerized tomography (CT) showed partial pneumothorax on the right, bilateral parenchymal cystic spaces and accompanying pericystic reticular pattern. Bronchoscopy was normal and bronchoalveolar lavage sample was non-specific. Videothoracoscopic wedge biopsy was performed and the immunohistochemical examination showed LCH. No sign of extra-pulmona ry involvement was found in the patient's body screening. Our patient was accepted as isolated pulmonary LCH. He was treated only by to quit smoking and there is no evidence of progression...

    LCH, also formerly known as eosinophilic granuloma or histiocytosis X, is characterized by impairment of the structure and functions of normal tissues by abnormal proliferation of Langerhans cells.     Extra nodal involvement areas such as gastrointestinal, head and neck, orbital, central and peripheral nervous system, thorax, bone, skin, breast, testis, thyroid and genitourinary system can be seen in 25-40% of HL and especially NHL patients, although it is known as lymph node malignancy (1,2). Extra nodal involvement is important in terms of prognosis. Imaging procedures play a very important role in diagnosis. 18F-FDG PET-CT imaging is a standard method in lymphoma patients and has a special importance in these patients in terms of superiority of CT imaging in detecting extra nodal regions. The role of 18F-FDG PET-CT in detection, staging and restaging of patients with extranodal involvement in NHL has also been reported in the literature (3,4). In this case presentation, in addition to nodal and mediastinal involvement areas in a patient diagnosed as NonHodgkin lymphoma, rare bilateral renal extra nodal simultaneous involvement detected on 18F-FDG PET-CT imaging were presented.

METHODS The study included 34 patients (27 males, 7 females; mean age 41 years; range 15-61 years) who underwent treatment for flail chest.     Malignant pleural mesothelioma is an aggressive type of malignancy which has a poor prognosis and for which no single protocol that can serve as the standard treatment could be established. Extrapleural pneumonectomy (EPP) can be curative in cases who are in the earlier stages of the malign pleural mesothelioma. In this case report, we present the experience we gained in the process of treating an early graft and intracavitary infection in a patient who underwent standard EPP and in whom grafts were applied to diaphragm and pericardium resections.

    Objectives: To evaluate the role of 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) for the diagnosis of anterior mediastinal masses. Methods: The oncological 18F-FDG PET/CT images of 41 patients (17 women, 24 men; age: 16-83 years, mean age: 50.5±19.5 years) who attended the nuclear medicine department between November 2016 and September 2017 were retrospectively evaluated for the metabolic characterization of their anterior mediastinal masses. Results: Based on our results, the lesions of 4 patients were benign [maximum standard uptake value (SUVmax) <3] and that of 2 patients were non-tumoral (i.e., tuberculosis and sarcoidosis). The mean dimensions and the SUVmax levels of the malignant lesions were 6.4±3.7 cm and 11.9±9.6, respectively.     Background This study aims to evaluate the efficiency of oleanolic acid on acute lung injury and acute respiratory distress syndrome. Methods The study included 70 female Wistar albino rats (weighing 180 to 200 g). We created seven groups, each consisting of 10 rats. Then, we generated acute lung injuries by intra-tracheal peroxynitrite injection in every group except for the control group. We investigated the effect of oleanolic acid. For this purpose, we measured the levels of malondialdehyde, interleukin 1 beta, interleukin 4, interleukin 10 and tumor necrosis factor alpha in the collected blood samples from the rats. In addition, we examined the lung tissue samples histopathologically and assessed the rate of apoptosis. Results Peroxynitrite injected groups at 24 and 48 h showed a statistically significant increase in interleukin 1 beta, tumor necrosis factor alpha, interleukin 4, interleukin 10 and malondialdehyde levels, which are accepted as mediators of the inflammatory process, compared to the control group. When peroxynitrite injected groups at 24 and 48 h were compared to the treatment groups of the same hour, a statistically significant decrease was detected. According to histopathological examination, peroxynitrite injected groups at 24 and 48 h showed a significant increase of tissue injury scores compared to the control group. However, the groups that were treated with oleanolic acid showed a significant decrease compared to the peroxynitrite groups (p<0.001 for tumor necrosis factor alpha and apoptosis results at 48 h). Conclusion In this study, we confirmed that oleanolic acid can be an effective agent for the prevention of acute lung injury generated via peroxynitrite.

    In this study, octreotide (OCT), a synthetic somatostatin analog, was tested for its beneficial effects in the prevention of interstitial pulmonary fibrosis (IPF) induced by bleomycin (BLM) in rats by histological examination and by evaluating tissue OH-proline levels. Thirty male Wistar rats were divided randomly into three groups: group I: intratracheal (i.t.) BLM (7.5 mg/kg, single dose) + saline solution [0.9% NaCl, subcutaneously (s.c.), once-daily for 7 days]; group II: i.t. BLM (7.5 mg/kg, single dose) + OCT acetate (82.5 µg/kg, s.c., once-daily for 7 days); and the control group. At the end of the 7 days, lung tissues were excised and examined by histopathological methods. Levels of tissue hydroxyproline (OH-proline) were determined. BLM administration resulted in prominent histopathologic findings, such as diffuse alveolar damage and interstitial pulmonary fibrosis, as well as a significant increase in OH-proline level, as compared to controls. OCT application explicitly attenuated the histopathologic changes to a significant extent. OCT decreased paranchymal fibrosis and structural deformities in BLM-induced lung fibrosis. These results suggest that OCT administration to rats with BLM-induced IPF has a protective effect. Further studies are necessary to reveal the molecular mechanism(s) of OCT-induced protective effect.
