The Psychological Impact of the SARS Epidemic on Hospital Employees in China: Exposure, Risk Perception, and Altruistic Acceptance of Risk

Abstract

Objective:

We examined the psychological impact of the 2003 outbreak of severe acute respiratory syndrome (SARS) on hospital employees in Beijing, China.

Methods:

In 2006, randomly selected employees (n = 549) of a hospital in Beijing were surveyed concerning their exposure to the 2003 SARS outbreak, and the ways in which the outbreak had affected their mental health.

Results:

About 10% of the respondents had experienced high levels of posttraumatic stress (PTS) symptoms since the SARS outbreak. Respondents who had been quarantined, or worked in high-risk locations such as SARS wards, or had friends or close relatives who contracted SARS, were 2 to 3 times more likely to have high PTS symptom levels, than those without these exposures. Respondents' perceptions of SARS-related risks were significantly positively associated with PTS symptom levels and partially mediated the effects of exposure. Altruistic acceptance of work-related risks was negatively related to PTS levels.

Conclusions:

The psychological impact of stressful events related to an infectious disease outbreak may be mediated by peoples' perceptions of those events; altruism may help to protect some health care workers against these negative impacts.

Objectif:

Nous avons examiné l'effet psychologique de l'éclosion du syndrome respiratoire aigu sévère (SRAS) de 2003 sur les employés d'un hôpital de Beijing, en Chine.

Méthodes:

En 2006, des employés (n = 549) d'un hôpital de Beijing, sélectionnés au hasard, ont été interrogés au sujet de leur exposition à l'épidémie de SRAS de 2003, et de la façon dont l'épidémie avait affecté leur santé mentale.

Résultats:

Environ 10 % des répondants avaient présenté des niveaux élevés de symptômes de stress post-traumatique (SPT) depuis l'épidémie de SRAS. Ceux qui avaient été mis en quarantaine, ou qui travaillaient dans des endroits à risque élevé, comme les unités du SRAS, ou qui avaient des amis ou des parents ayant contracté le SRAS, étaient de 2 à 3 fois plus susceptibles d'avoir des niveaux élevés de symptômes de SPT que ceux qui n'avaient pas été ainsi exposés. Les perceptions des répondants des risques liés au SRAS étaient significativement positivement associées aux niveaux de symptômes de SPT, et facilitaient partiellement les effets de l'exposition. L'acceptation altruiste des risques liés au travail était négativement liée aux niveaux de SPT.

Conclusions:

L'effet psychologique d'événements stressants liés à l'épidémie d'une maladie infectieuse peut être facilité par les perceptions d'une personne de ces événements; l'altruisme peut contribuer à protéger certains prestataires de soins de santé contre ces effets négatifs.
