Optic Neuritis

PURPOSE: To describe retinal nerve fiber layer changes in late-stage diffuse unilateral subacute neuroretinitis eyes and compare these results with healthy eyes observed through nerve fiber analyzer (GDx®). METHODS: This is a retrospective case-control study in which 49 eyes in late-stage diffuse unilateral subacute neuroretinitis were examined from May/97 to December/01. First, eyes with diffuse unilateral subacute neuroretinitis and healthy contralateral eyes (Control Group I) were statistically matched. Subsequently, eyes with diffuse unilateral subacute neuroretinitis were compared with eyes of healthy patients (Control Group II). RESULTS: Eyes from Control Groups I and II had higher relative frequency of "within normal limits" status. Eyes from the diffuse unilateral subacute neuroretinitis (DUSN) Group had higher frequency of "outside normal limits" and "borderline" status. Control Groups I and II had absolute values different from the DUSN Group regarding all parameters (p<0.05), except for Symmetry in Control Groups I and II, Average thickness and Superior Integral in control group II. CONCLUSION: Patients with late-stage diffuse unilateral subacute neuroretinitis presented presumed decrease in nerve fiber layer thickness shown by GDx®. Retinal zones with larger vascular support and larger amount of nerve fibers presented higher decrease in the delay of the reflected light measured by the nerve fiber analyzer.

Atrophy demarcating to musculus teres minor is seen in magnetic resonance imaging of the shoulder region in 3 to 5.5% of investigations. We describe seven patients with prolonged or recurrent pain of the shoulder region, who were diagnosed in ENMG with damage of the axillary nerve brand to m. teres minor. Probable causes included tear of the rotator cuff, luxation of the shoulder joint, nerve entrapment in the quadrilateral space, neuritis of the brachial plexus and iatrogenic damage associated with shoulder arthroscopy. This nerve damage cannot be proved clinically.

Polyneuritis cranialis is a rare disorder of multiple cranial nerve palsies without spinal cord involvement. The case reported is of a 10-year-old boy with multiple palsies involving cranial nerves III, IV, V, VI, VII, IX, X, XI, and XII cranial nerves, unilaterally or bilaterally. After initial, unsuccessful treatment, corticosteroids and acyclovir were administered. The patient dramatically improved after two cycles of intravenous immunoglobulins. He was left with a mild residual left VI nerve deficit, which fully remitted after 2 months.

Background: Optic neuritis is an inflammation of the optic nerve. Because of importance of vision
for human, management of optic neuritis is an important issue in neurology.
Objectives: This study aimed at assessing the effect of oral steroid treatment after intravenous (IV)
methylprednisolone therapy on demyelinating optic neuritis.
Materials & Methods: 60 Patients with first episode of acute demyelinating optic neuritis
who referred to a neurology clinic in an academic hospital in south-west of Iran in 2015-2016
included in this randomized double-blind clinical trial study (No: IRCT2015102724735N1). They
were divided into two groups with (A) and without (B) tapering oral steroid treatment after IV
methylprednisolone. Visual acuity and color vision were measured before treatment, before oral
tapering, 1 and 3 months after treatment. Independent t-test in SPSS software version 20 was used
to analyze the data. P<0.05 was considered as the level of significance.
Results: Visual acuity in two groups did not have any significant difference in any time point of
assessment (P>0.05).Frequency of color vision abnormality in groups A and B were at baseline
(53.3% vs. 56.6%) (P=0.796), before oral tapering (43.3% vs. 30%, P=0.284), 1 month after
treatment (23.3% vs. 30%, P=0.559), 3 months after treatment (0% vs. 23%, P=0.011).
Conclusion: Oral steroid treatment after IV methylprednisolone pulse therapy improves color
vision after 3 months in patients with demyelinating optic neuritis but has no effect on visual acuity.
