Write-up: Patient asked his age and said he was 49 yrs old and completed a questionnaire which asked if he was younger than 2 yrs old or older than 49 yrs old and he again answered no. The computer system was down at the time so he registered on paper and, as the nurse vaccinating, I reviewed his answers to the questions but did not double check his birthdate to confirm his age was between 2-49. He was administered the FluMist intranasally. Called him today to inform him of the error and he said he tolerated both vaccines well and is not experiencing any side effects.

Write-up: I had an itchy, painful shingles rash on the left side of my back and left torso. I started having bad nerve pain in my back. I also so had fatigue and loss of appetite. I went to the doctor, and I was diagnosed with shingles, and they prescribed me an antiviral. On the 10th I went to the urgent care because I had kidney stones, and they did a CAT scan to confirm.

Write-up: I tested positive for COVID-19 on 12/29/2022. I had a mild cough with production, and a little bit of a runny nose and some fatigue. I took Paxlovid starting 12/30/2022. My doctor prescribed that for me and I had the worst taste in my mouth. In general, my COVID-19 infection was very mild. I didn''t really think I had it. My wife had the infection too that was more substantial than mine. We took the COVID-19 tests at home to confirm. In general, I''ve always had very strong reactions to the vaccines, so I believe my immune system was ready to go when the time came. About a month after my COVID-19 infection, I had an acute pancreatitis attack. On 02/04/2022, I had acute pancreatic attack with thrushing abdominal pain and back pain. Part of that, I basically had a lack of appetite that went back to my COVID-19 infection. My appetite reduced and my stool was loose for that period of time. I was hospitalized for 3 days and they treated me with antibiotics and bland diet. The bland diet is still ongoing. I''ve recovered from the COVID-19 infection, but I''m still recovering from the pancreas health event.

Write-up: My wife started having a slight headache, head congestion, sore throat and coughing. A few days later I started having the same symptoms three days later. I did a at home COVID-19 test and, I was positive. I did a telehealth visit and was given Paxlovid. All my symptoms went away I think the worse was a fever and a slight cough. The treatment did help. I am still having some congestion and lethargic.

Write-up: Beginning of November, I tested positive for COVID-19. My first symptoms that all a stuffy head. And it felt like a really bad, I was tired with congestion. I did go to the doctor for Paxlovid. But the in the middle of February I came down with a cold and was tested for FLU, COVID-19 and Strep. All were negative. They couldn?t get anything because it was not bacterial. That night I could barely breathe while resting. I went back and was given a Z Pack and Prednisone and a cough syrup. I?m sure these medications kept this from forming in pneumonia.

Write-up: I was having a cough, like a bronchitis cough. It wasn''t alot of mucus or chest compression. I was two or two or three days before I decided to test. I did have a slight elevated temperature. I tested with a at home COVID-19 test and it was positive. I called my dotor and they prescribed Paxlovid. During the time of taking it my symptoms were better.
