Objectives.  Our report describes the construction and evaluation of the Arabic Index Premature Ejaculation (AIPE) as a diagnostic tool for premature ejaculation (PE) and presents data supporting its validity.Methods and Main Outcome Measures.  Seventy-one men complaining of PE and 73 healthy subjects were asked to complete the seven-question AIPE. Diagnosis of PE was based on the criteria set by the second consultation on sexual dysfunctions. The seven items selected were based on assessment of erectile function, sexual desire, ejaculation latency, ejaculation control, patient satisfaction, partner satisfaction, and psychological distress. The AIPE was examined for sensitivity, specificity, and construct validity.Results.  A receiver operating characteristic curve indicated that the AIPE is an excellent diagnostic test. A cutoff score of 30 (range of scores 7–35) discriminated best (sensitivity = 0.98, specificity = 0.88). Severity of PE ranged from none (31–35) to severe (7–13). A high kappa value (0.85) indicated existence of significant agreement existed between the predicted and “true” PE classes.Conclusions.  AIPE shows a potential to be a reliable aid to decrease the number of misdiagnosed cases of PE. The occurrence of a distinctive EEG pattern specifically related to sexual arousal and orgasm would provide a reliable and convenient means of identifying such events in the laboratory and would also provide clues to cerebral structures involved in the processes. EEG-polygraph recordings were obtained under rigorously controlled conditions in four normal male subjects during masturbation and ejaculation. The EEG data were subjected to both impressionistic and quantitative analyses. They showed no remarkable changes during the sequence of relevant physiological responses. The sole effect was a slight depression of alpha activity, a well-known nonspecific effect associated with changes in attention and arousal. Examination of the literature shows little agreement among reported results of studies of EEG changes during orgasm. It is likely that at least some reported changes were artifactual. It is concluded that the case for the existence of EEG changes specifically related to sexual arousal and orgasm remains unproven.

Although semen quality is one of the major traits that influence breeding soundness examination outcomes in bulls, field conditions occasionally do not allow for the collection of semen samples by means of an artificial vagina. The aims of the present study were to report the results of a large number of semen collections that were performed via the transrectal massage (TRM) of the accessory sexual glands of Italian yearling beef bulls and compare this semen collection method to the artificial vagina (AV) method in term of breeding soundness examination outcomes; furthermore, we determined whether the breed affected the semen characteristics. In the TRM group (n = 475), the semen samples were collected via TRM of the accessory sexual glands, and in the AV group (n = 502), the AV method was used. In the TRM group, semen samples were obtained from 81.3% of the bulls and penile protrusion was observed in 87.6% of the animals during semen collection. The sperm concentrations (920.5 ± 439.0 vs. 281.0 ± 259.8 × 10(6)/mL) and the percentages of total abnormal spermatozoa (22.8 ± 15.0 vs. 18.8 ± 12.9) were significantly higher in the AV group than those in the TRM group. The percentage of bulls that did not meet the minimum requirement for normal cells (≥70%) was 6.2% higher in the AV group than that in the TRM group (P < 0.05). Moreover, the samples collected from Chianina bulls by TRM exhibited a lower percentage of motile sperm and a higher percentage of abnormal spermatozoa when compared with the other two breeds. The major drawbacks of the TRM technique were the inability to conduct complete evaluation of the libido and mating ability of the yearling bulls, a significant reduction of the number of spermatozoa collected, and an increase in the variability of the semen characteristics due to breed. In conclusion, despite the drawbacks, TRM guarantees that semen evaluation can be conducted in cases in which the semen samples cannot be collected with the AV method.

We secured data from 243 men in committed, sexual, heterosexual relationships to test the sperm retention hypothesis of oral sex. We predicted that, among men who perform cunnilingus on their partner, those at greater risk of sperm competition are more likely to perform cunnilingus until their partner achieves orgasm (Prediction 1), and that, among men who ejaculate during penile-vaginal intercourse and whose partner experiences a cunnilingus-assisted orgasm, ejaculation will occur during the brief period in which female orgasm might function to retain sperm (Prediction 2). The results support Prediction 1 but not Prediction 2. We discuss limitations of the current research and discuss how these results may be more consistent with alternative hypotheses regarding female orgasm and oral sex.

Male Indian Flying Foxes (Pteropus giganteus) that spend more time performing oral sex on a female also spend more time copulating with her. In humans, men who spend more time copulating with their regular partner also perform more "semen-displacing" copulatory behaviors (e.g., deeper, more vigorous penile thrusting). We investigated whether men who spend more time performing oral sex on their regular partner also spend more time copulating with her and perform more semen-displacing copulatory behaviors. We proposed and tested the ejaculate adjustment hypothesis for men's copulatory behaviors: Men adjust their copulatory behaviors to increase their sexual arousal and consequent ejaculate quality, thereby increasing their chances of success in sperm competition. Dysejaculation following groin hernia repair can occur in about 1-2% of patients, resulting in impairment of sexual function. We report a case of chronic postherniotomy dysejaculation treated with transection of the ilioinguinal and iliohypogastric nerves and decompression of vas deferens that was embedded and twisted in shrunken mesh and scar tissue. At three months follow-up, there was reduced overall pain and no dysejaculation, and quantitative sensory testing showed reversal of sensory abnormalities, except for sensory loss, compared with preoperative values.
