This report describes the variability in the mechanism of upper airway collapse seen in children with obstructive apnea secondary to craniofacial anomalies. Emphasis is on the nasopharyngoscopic observation of the upper airway and the accurate assessment of the site and mechanism of obstruction in order to prescribe the appropriate treatment.

BACKGROUND Functional appliances have been used since many decades for the correction of mandibular retrognathism. Management of oropharyngeal swallowing dysfunction often requires both a clinical and an instrumental examination. A videofluoroscopic swallowing study is an instrumental examination that often could be a good option and a very useful tool for the swallowing clinician. At Skåne University Hospital, Malmö, Sweden, the name of such examination is therapeutic videoradiographic swallowing study (TVSS). A TVSS examination should always be performed in collaboration between a speech language pathologist and a radiologist. During the examination, the patient is seated in an upright position, but the examination can also be performed with the patient lying down. The TVSS examination can be performed both in frontal and lateral projection. Test material with varied consistencies as well as different therapeutic strategies can be tested during the examination. To evaluate the role of lateral skull base approaches in the management of benign parapharyngeal space tumors and to propose an algorithm for their surgical approach. Retrospective study of patients with benign parapharyngeal space tumors. The clinical features, radiology and preoperative management of skull base neurovasculature, the surgical approaches and overall results were recorded. 46 patients presented with 48 tumors. 12 were prestyloid and 36 poststyloid. 19 (39.6%) tumors were paragangliomas, 15 (31.25%) were schwannomas and 11 (23%) were pleomorphic adenomas. Preoperative embolization was performed in 19, stenting of the internal carotid artery in 4 and permanent balloon occlusion in 2 patients. 19 tumors were approached by the transcervical, 13 by transcervical-transparotid, 5 by transcervical-transmastoid, 6, 1 and 2 tumors by the infratemporal fossa approach types A, B and D, respectively. Total radical tumor removal was achieved in 46 (96%) of the cases. Lateral skull base approaches have an advantage over other approaches in the management of benign tumors of the parapharyngeal space due to the fact that they provide excellent exposure with less morbidity. The use of microscope combined with bipolar cautery reduces morbidity. Stenting of internal carotid artery gives a chance for complete tumor removal with arterial preservation.

The purpose of this study was to analyze the morphologic changes of the airway in obstructive sleep apnea (OSA) patients by helical computed tomography (CT) scanning following maxillomandibular advancement (MMA) surgery. Twenty consecutive OSA patients treated with MMA from 2000 to 2003 at the University of Alabama at Birmingham Hospital and diagnosed by polysomnography (PSG) were included in this study. The criteria for this study were MMA surgery, pre- and postoperative helical CT scan, and PSG. Lateral (LAT) and anterioposterior (AP) airway dimensions were measured from the level of the hard palate to the hyoid using helical CT scans. There was enlargement of LAT and AP diameters for all patients at all levels. LAT dimensions were enhanced greater than AP in the retroglossal region. MMA results in significant increase in both AP and LAT airway dimensions.

Objective Characterize complications following uvulopalatopharyngoplasty (UPPP) for obstructive sleep apnea. Study design Retrospective chart review. Subjects and methods Charts of patients undergoing UPPP at an academic teaching hospital from 1999 to 2005 were reviewed. Results 345 consecutive patients (248 inpatients; 97 outpatients) were studied. The most common post-operative complication in the entire study was oxyhemoglobin desaturation (12.8%). Three patients suffered major complications (airway obstruction, pulmonary edema, arrhythmia). Regarding complications limited to the post-anaesthetic care unit alone, only 8.2% of patients had oxyhemoglobin desaturation after discontinuation of oxygen supplementation. Inpatients requiring supplemental oxygen on the ward had significantly higher mean AHI (37.4 vs. 31.4; p=0.05) and BMI (32.3 kg/m2 vs. 28.9 kg/m2; p=0.004) than those who did not.
