Doorways to Care

Doorways to Care is an enhanced phone-based program helping seniors connect with local community-care organizations.

Our “live answer” service can assist you to find the services you need, speaking your language, and helping you and your loved ones to coordinate care. 

By calling the toll free line (or by speaking to a staff person within one of the participant agencies) you can apply for the services of any of the agencies involved. 

You can find a wide range of services available within the Doorways to Care organizations including, delivered meals, transportation, adult day programs, home support, nursing, rehabilitation therapy, personal support, caregiver respite and more!

Who We Serve

The Doorways to Care Network was established to support seniors to live safely and independently in the community. Collectively, we serve:

• People age 65 and older
• People age 55 and older with an age-related condition (such as Parkinson’s or Alzheimer’s disease)
• Caregivers for seniors (e.g. family, friends)

Our goal is to make access to health and community services easier for seniors, caregivers and families who live within the Central Local Health Integration Network (LHIN) catchment area.
