This study evaluates the effectiveness of a performance-focused methodology for engaging multidisciplinary, frontline healthcare teams in making behavioural changes that improve patient care and health system efficiency. Results include significant declines in average length of stay in hospital and waiting time for surgery, and a dramatic increase in early patient ambulation. Performance-focused methodology using key performance indicators, targets, measurement, and ongoing feedback, supported by non-monetary incentives, can quickly improve healthcare outcomes.

We describe the design of an object-oriented multimedia database management system that can store and manage SGML/HyTime compliant multimedia documents. The system is capable of storing, within one database, different types of documents by accommodating multiple document type definitions (DTDs). This is accomplished by dynamically creating object types according to element definitions in each DTD. The system also has tools to automatically insert markedup documents into the database. We discuss the system architecture, design issues and the system features. 1. Background There are limited data on the epidemiology of adolescent soccer injury across all levels of play. Hypothesis Through implementation and validation of an injury surveillance system in adolescent soccer, risk factors for injury will be identified. Study Design Descriptive epidemiology study. Methods The study population was a random sample of 21 adolescent soccer teams (ages 12-18). A certified athletic therapist completed preseason baseline measurements and did weekly assessments of any identified soccer injury. The injury definition included any injury occurring in soccer that resulted in 1 or more of the following: medical attention, the inability to complete a session, or missing a subsequent session. Results Based on completeness of data in addition to validity of time loss, this method of surveillance has proven to be effective. To determine prevalence and incidence of carpal tunnel syndrome (CTS) in a modern meat packing plant. The secondary objective was to explore the relation between ethnicity and CTS. Six hundred and sixty five workers were interviewed and examined to find the prevalence of CTS. Subsequently, 421 workers without CTS were followed up and examined at a median interval of 253 days; of those, 333 remained without CTS and were again examined at a median interval of 148 days. The prevalence and incidence of CTS was 21% and 11/100 person-years, respectively. The incidence for Asian mixed, white, and other ethnicities was 12.0, 12.2, and 7.2 cases/100 person-years, respectively. The observed incidence for men and women was 9.7 and 18.4 cases/100 person-years, respectively. This difference was not quite significant (p = 0.068) with an estimated relative risk (women v men) of 1.9 (95% confidence interval (95% CI) 0.9 to 3.8). The interaction between sex and use of tools was significant (p = 0.04), however, although the relative risk for CTS in women who used tools was 4.2 the numbers were small and not significant. The relative risk for men who used tools was 0.64 and not significant. The percentage of incident cases with comorbid disease was only 6.3% (3/47). The prevalence and incidence of CTS in this workforce were higher than in the general population. However, the prevalence of CTS in this modern, mechanised plant was not significantly different from that reported in older plants. No relation was found between ethnicity, age, body mass index, and CTS for either prevalence or incidence. Comorbid disease among the cases of CTS is significantly less than that found in other industry.

An increasing proportion of those living with single ventricle physiology have hypoplastic left heart syndrome (HLHS). Our objective was to assess the association between HLHS and outcomes post Fontan operation. All pediatric patients who underwent a Fontan procedure at the University of Alberta between 1996 and 2016 were included. Follow-up clinical data collected included early and late surgical or catheter reintervention, echocardiography, and long-term transplant-free survival. Characteristics were compared between those with and without HLHS, and the association between outcomes and HLHS were assessed. A total of 320 children (median age 3.3 years, interquartile range 2.8 to 3.9 years; 121 [43.4%] female) underwent a Fontan procedure over the course of the study. Patients with bone metastases may experience pain, fatigue, and decreased mobility. Multiple medications for analgesia are often required, each with attendant side effects. Although palliative-intent radiotherapy (RT) is effective in decreasing pain, additional supportive care interventions may be overlooked. Our objective was to describe the feasibility of multidisciplinary assessment of patients with symptomatic bone metastases attending a dedicated outpatient palliative RT clinic. Consecutive patients referred for RT for painful bone metastases were screened for symptoms and needs relevant to their medications, nutritional intake, activities of daily living, and psychosocial and spiritual concerns from January 1 to December 31, 2007. Consultations by appropriate team members and resulting recommendations were collected prospectively. Patients who received RT were contacted by telephone 4 weeks later to assess symptom outcomes. A total of 106 clinic visits by 82 individual patients occurred. As determined by screening form responses, the clinical Pharmacist, Occupational Therapist, Registered Dietician and Social Worker were consulted to provide assessments and recommendations within the time constraints presented by 1-day palliative RT delivery. In addition to pain relief, significant improvements in tiredness, depression, anxiety, drowsiness and overall well-being were reported at 4 weeks. Systematic screening of this population revealed previously unmet needs, addressed in the form of custom verbal and written recommendations. Multidisciplinary assessment is associated with a high number of recommendations and decreased symptom distress. Our findings lend strong support to the routine assessment by multiple supportive care professionals for patients with advanced cancer being considered for palliative RT.

Poverty influences health status, life expectancy, health behaviours, and use of health services. This study examined factors influencing the use of health-related services by people living in poverty. In the first phase, 199 impoverished users of health-related services in 2 large Canadian cities were interviewed by their peers. In the second phase, group interviews with people living in poverty (n = 52) were conducted. Data were analyzed using thematic content analysis. Diverse health-related services were used to meet basic and health needs, to maintain human contact, and to cope with life's challenges. Use of services depended on proximity, affordability, convenience, information, and providers' attitudes and behaviours. Use was impeded by inequities based on income status. Sulfide accumulation due to bacterial sulfate reduction is responsible for a number of serious problems in the oil industry. Among the strategies to control the activity of sulfate -reducing bacteria ( SRB ) is the use of nitrate, which can exhibit a variety of effects. We investigated the relevance of this approach to souring oil fields in Oklahoma and Alberta in which water flooding is used to enhance oil recovery. SRB and nitrate -reducing bacteria ( NRB ) were enumerated in produced waters from both oil fields. In the Oklahoma field, the rates of sulfate reduction ranged from 0.05 to 0.16 M S day À 1 at the wellheads, and an order of magnitude higher at the oil -water separator. Sulfide production was greatest in the water storage tanks in the Alberta field. Microbial counts alone did not accurately reflect the potential for microbial activities. The risk of soil acidification is high in the Athabasca oil sands region (AOSR) in Alberta, Canada, due to elevated SO2 emission and the resultant acid deposition to sensitive, coarse-textured soils. An implementation and one-year follow-up of the Gambling Decisions program attempted to answer several important questions. First, is controlled gambling a viable treatment option for some gamblers? Can earlier stage problem gamblers be separated for treatment from those with more severe problems? Finally, would problem gamblers utilize a community health agency for treatment of their excessive gambling? A pretest/posttest design was chosen where the efficacy of the program was assessed using repeated measures ANOVA analysis. Results showed that an average loss of $608 over a 4-week period was reduced to $113 immediately after the 6-week program and to a loss of $73 at 12 months. The average number of hours spent gambling per 4 weeks was significantly reduced from 23.5 at pretest to 6.5 at the 12 month posttest. This report aims to provide a summary of the historical, cultural, and policy context within which the Sixties Scoop developed and was addressed in Alberta. Alberta was not unique in the development of its child welfare policies with respect to Indigenous peoples. Very similar trajectories of slow institutional reform, missteps and tragedies unfolded in comparable jurisdictions such as Saskatchewan (Stevenson, 2013) and very different
contexts such as British Columbia (Smith, 2009). The report will discuss some similarities and distinctions with other jurisdictions in Canada in order to situate Alberta in the broader context. Within the Province of Alberta there are three treaty territories (Treaty 6, 7, and 8), 140 reserves, and eight Métis settlements. A number of Indigenous communities
reside in Alberta, including Blackfoot, Cree, Dene, Métis, and Nakoda (Stoney). Due to Alberta’s particular legal and political history, these groups were uniquely impacted by Alberta’s Sixties Scoop policies and practices. The issue admits of considerable urgency given that Alberta continues to have one of the highest proportions of Indigenous children and youth in various child welfare and juvenile institutions (Alberta Office of the Child
and Youth Advocate, 2016).

Palms (Arecaceae) are iconic plant fossils, providing evidence of warm climates in the geological past in geographical areas that today support temperate, boreal or even polar climates. Fossil palm leaves are well known from Paleocene sites in the U.S.A., including Montana, Washington, and Alaska. Palm megafossils are unknown, however, from the Canadian Paleocene. Here, palms from the Scollard Formation of Alberta are described and illustrated from the early Paleocene Genesee locality, central Alberta. These fossil palm leaves are fan palms (Subfamily Coryphoideae), but the limited diagnostic information of leaf fragments prevents assignment to a palm tribe. The Genesee megaflora represents a broad-leaf deciduous boreal forest growing under a temperate moist climate. The Genesee palm fossils demonstrate the northernmost Paleocene palms known from leaf fossils east of the Rocky Mountains, and are tiny at b 15 cm, likely reflecting plants growing at their cold northern limit.

This document will discuss the profitability of mining Bitcoin (BTC) at the consumer level through analysis of electricity prices, forecasted Bitcoin prices, network difficulty, and current hardware. A breakeven and cash-flow analysis determines the viability of mining and explores potential future scenarios. Finally, the risks associated with mining are evaluated with respect to current and future feasibility.

OBJECTIVES To estimate the life expectancy and specify the causes of death among people with fetal alcohol syndrome (FAS). METHODS Included were all patients recorded in Alberta provincial databases of inpatients, outpatients, or practitioner claims from 2003 to 2012. People with FAS were identified by ICD-9 code 760.71 and ICD-10 codes Q86.0 and P04.3, and were linked to the Vital Statistics Death Registry to get information about mortality. Life expectancy was estimated by using the life table template developed in the United Kingdom, which is recommended for estimating life expectancy in small areas or populations. RESULTS The life expectancy at birth of people with FAS was 34 years (95% confidence interval: 31 to 37 years), which was about 42% of that of the general population. Although perfectionist orientations have been linked to a variety of cognitive, affective, and behavioral correlates in youth sport, little is known about antecedent factors that may influence adolescent athletes' perfectionist orientations. The purpose of this study was to determine whether perceptions of parenting styles differ as a function of adolescent athletes' perfectionist orientations. A total of 194 male youth soccer players (M age = 13.64 years; SD = 1.51; range, 10.67-16.25 years) completed measures of their perfectionist orientations in sport and of their perceptions of maternal and paternal parenting styles. Scores from the parenting style measure were calculated such that higher scores were reflective of higher parental authoritativeness (as perceived by the athletes).
