The aim of this study has been to assess the larynx and soft tissue around the vocal tract in a group of people with healthy voice, and in a group of patients with occupational dysphonia using the new laryngeal manual therapy palpatory evaluation scale (LMTPE). The examinations were performed in a study (dysphonic) group of professional voice users who had developed voice disorders (N = 51) and in the control group of normophonic subjects (N = 50). All the participants underwent perceptual voice assessment and examination by means of the LMTPE scale. Objective: To assess the intraexaminer and interexaminer reliability of clinicians trained in flexion-distraction technique to determine the need for chiropractic adjustment of each segment of the lumbar spine. Design: This was an intraexaminer and interexaminer reliability study of commonly used chiropractic assessment procedures, including static and motion palpation and visual observation. Setting: Chiropractic college; by four licensed chiropractors trained in flexion-distraction technique, two with more than 20 years' experience and two with 3 or fewer years' experience. Subjects: Subjects were 18 volunteers; 16 were symptom free, and 2 had low back pain at the time the study was conducted. Main Outcome Measure: The kappa statistic was computed for all comparisons and interpreted in categories ranging from “poor” (<0.00) to “almost perfect” (>0.80). Results: Intraexaminer reliability was greater than interexaminer reliability. For intraexaminer reliability there was considerable variation by segment and among the four examiners, but intraexaminer reliability appeared generally higher than interexaminer reliability. Overall, more subluxations were identified on the second examination than on the first. For interexaminer reliability, kappa scores were generally in the “poor” to “slight” categories. Discussion: The results of this study, similar to those of other studies, indicate that even chiropractors trained in the same technique seem to show little consensus on the indications for the necessity to adjust specific segments of the spine. The objectives of the current study were to analyze the effects of orofacial myofunctional therapy (OMT) on the treatment of subjects with associated articular and muscular temporomandibular disorders (TMD). Thirty subjects with associated articular and muscular TMD, according to the Research Diagnostic Criteria (RDC/TMD), were randomly divided into groups: 10 were treated with OMT (T group), 10 with an occlusal splint (OS group), and 10 untreated control group with TMD (SC). Ten subjects without TMD represented the asymptomatic group (AC). The potency of touch in osteopathic manipulative treatment (OMT) is physically realized within the musculoskeletal, immune, nervous, and endocrine systems. Psychologically, touch supports a verbal and tactile interaction that is both diagnostic and therapeutic. Touch is a 2-way street that adds meaning and depth to the patient-physician experience. The relationship between touching and being touched offers a potentially powerful and intense deepening of the patient-physician relationship that emerges within the palpatory examination and treatment. Empathic communication, through word or deed, allows a therapeutic, synchronized healing to occur. In the present article, the authors provide a rationale to sensitize and invigorate osteopathic physicians to routinely evaluate and treat patients using their skillful touch.

"This thesis examines the extent to which the development of expertise in diagnostic palpation in osteopathic medicine is associated with changes in cognitive processing. Chapter 2 and Chapter 3 review, respectively, the literature on the role of analytical and non-analytical processing in osteopathic and medical clinical decision making; and the relevant research on the use of vision and haptics and the development of expertise within the context of an osteopathic clinical examination.

The two studies reported in Chapter 4 examined the mental representation of knowledge and the role of analogical reasoning in osteopathic clinical decision making. The results reported there demonstrate that the development of expertise in osteopathic medicine is associated with the processes of knowledge encapsulation and script formation. The four studies reported in Chapters 5 and 6 investigate the way in which expert osteopaths use their visual and haptic systems in the diagnosis of somatic dysfunction. The results suggest that ongoing clinical practice enables osteopaths to combine visual and haptic sensory signals in a more efficient manner. Such visuo-haptic sensory integration is likely to be facilitated by top-down processing associated with visual, tactile, and kinaesthetic mental imagery.

Taken together, the results of the six studies reported in this thesis indicate that the development of expertise in diagnostic palpation in osteopathic medicine is associated with changes in cognitive processing. Whereas the experts’ diagnostic judgments are heavily influenced by top-down, non-analytical processing; students rely, primarily, on bottom-up sensory processing from vision and haptics. Ongoing training and clinical practice are likely to lead to changes in the clinician’s neurocognitive architecture.

This thesis proposes an original model of expertise in diagnostic palpation which has implications for osteopathic education. Students and clinicians should be encouraged to appraise the reliability of different sensory cues in the context of clinical examination, combine sensory data from different channels, and consider using both analytical and non-analytical reasoning in their decision making. Importantly, they should develop their skills of criticality and their ability to reflect on, and analyse their practice experiences in and on action."

Breast elastography is a new sonographic technique that provides additional characterization information on breast lesions over conventional sonography and mammography. This technique provides information on the strain or hardness of a lesion, similar to a clinical palpation examination. Two techniques are now available for clinical use: strain (compression-based elastography) and shear wave elastography. Initial evaluation of these techniques in clinical trials suggests that they may substantially improve the characterization of breast lesions as benign or malignant. This improvement may substantially reduce the number of benign biopsies performed. Elastography can be performed by several methods and is now available from several manufactures. This article reviews the basics of this technique, how to perform the examination, image interpretation, and artifacts.
