Abstract

Myocarditis is an inflammatory condition affecting the myocardium, variably caused by infectious agents, autoimmune conditions, or reactions to exogenous/endogenous substances. The exact incidence of myocarditis is unknown. The clinical presentation is very heterogeneous and the disease remains an underdiagnosed cause of acute heart failure, sudden death, and chronic dilated cardiomyopathy. Endomyocardial biopsy is still considered the diagnostic gold standard but is not free of risks, can have false negative results, and is of variable diagnostic accuracy. As a consequence, many efforts have been recently made to improve a non-invasive diagnostic approach, exploiting cardiovascular magnetic resonance, positron emission tomography, and molecular imaging.
