Zusammenfassung
Der Begriff „infektiöse Endokarditis“ bezeichnet eine endovaskuläre, vorzugsweise durch Bakterien verursachte Infektion des Endokards der nativen Herzklappen, aber auch von intravaskulär implantierten Fremdmaterialien, wie Klappenprothesen oder Schrittmacherelektroden. Wegen der großen Heterogenität des klinischen Bildes der Endokarditis ist eine Einteilung in 4 Kategorien hinsichtlich des Ortes der Infektion und der Präsenz von intrakardialem Fremdmaterial empfehlenswert: 1. Nativklappenendokarditis der linken Herzhöhlen, 2. Kunstklappenendokarditis der linken Herzhöhlen, 3. Rechtsherzendokarditis und 4. Endokarditis in Zusammenhang mit intrakardialem Polymermaterial. Hinsichtlich der Akquisition der infektiösen Endokarditis lassen sich folgende Wege differenzieren: 1. ambulant erworbene Endokarditis, 2. durch medizinische Maßnahmen erworbene Endokarditis (nosokomiale und nichtnosokomiale Infektionen) und 3. infektiöse Endokarditis durch intravenösen Drogenabusus. Nach dem Verlauf kann eine subakute Endokarditis (Endokarditis lenta, meist Streptokokken) von einer akuten, schnell fortschreitenden Endokarditis (häufig Staphylokokken) unterschieden werden.
