Comment on "Autoantibodies against type I IFNs in patients with life-threatening COVID-19"
In their recent paper, Bastard et al (1) reported that 10% of patients with life-threatening COVID-19 pneumonia had autoantibodies against type I interferons, and that half of the patients with such autoantibodies were 65 years and older. Notably, anti-interferon autoantibodies were not detectable in the considerably younger individuals with asymptomatic or mild SARS-CoV-2 infection and in only 4 of 1227 of younger healthy individuals. The lack of age-matched cohorts poses a significant problem because the authors appear not to have ruled out the risk of false-positive results due to the possible presence of rheumatoid factors (RFs) and other heterophilic antibodies in the aged group of patients with life-threatening COVID-19 pneumonia.
RFs are antibodies directed against the Fc region of immunoglobulin G. These antibodies are particularly prominent in patients with rheumatoid arthritis but can also be found in patients with other autoimmune diseases, and in patients with certain infections, as well as in healthy subjects (2). The overall prevalence of RF in healthy individuals is approximately 4%, but the prevalence increases with age and is close to 30% in the elderly (3-5).
RFs constitute a well-documented problem for clinical laboratories (2). RFs present in serum and/or plasma interfere with many different antibody-based laboratory tests and can give rise to false-positive test results (6-11), a challenge for diagnostic laboratorians and for clinicians interpreting the test results.
Extrapolation of data from Fig S4 in the study by Bastard et al (1) show that the three study cohorts differ considerably in age. Approximately 27% of the patients with life-threatening COVID-19 were elderly (above 70 years of age) but only 2% of the healthy control subjects and 0% of the patients with mild COVID-19. Thus, the prevalence of anti-interferon autoantibodies show the same age-distribution pattern as is normally observed for RFs (3-5). Moreover, the immunoglobulin isotypes of these anti-interferon autoantibodies, as presented by Bastard et al in Figure S1 show that these antibodies were of the IgG, IgA, IgM or IgE isotypes, a feature they share with RFs (12, 13).
We conclude that data on the presence of RFs in the patient sera as well as technical information regarding potential interference by RFs in the analyses of autoantibodies to type I interferons are warranted for adequate interpretation of the data presented in the study by Bastard and co-workers.

Convalescent plasma: beyond COVID-19 neutralising antibodies
In the midst of worldwide COVID-19 pandemic, preliminary evidence, suggest that convalescent plasma may lessen the severity of COVID-19 (1-3). Recently, Bastard et al., (4) report that at least 10% of patients with life-threatening COVID-19 pneumonia have neutralizing auto-Abs against type I IFNs. The authors discuss that patients recovering from life-threatening COVID-19 should also be excluded from donating convalescent plasma for ongoing clinical trial, or at least tested before their plasma donations are accepted. Inflammatory responses have been reported after fter transfusion of all types of blood components, including red blood cells (RBC) (5), platelet concentrates (PC) (6) and fresh frozen plasma (FFP) (7); with FFP being the most involved blood product. Although not completely understood, the pathological mechanisms underlying transfusion-associated acute lung injury (TRALI) after transfusion of FFP or plasma-containing PC mainly are thought to result from an inflammatory response involving neutrophilic infiltration into the lungs and elevated interleukin (IL)-8 and IL-1 pulmonary levels (8, 9).
To provide relief of COVID-19 patients with severe acute respiratory syndromes, transfusion of convalescent plasma therapy, containing anti-SARS-CoV-2 polyclonal antibodies is being evaluated (10). Quality control for convalescent plasma currently include standard microbiological assessment for the absence of transfusion-transmissible infectious agents as well as quantification of anti-SARS-CoV-2 antibodies while other immune parameters, including inflammatory characteristics are not considered. Plasma transfusion may modulate innate immune responses (ref). Cytokines and chemokines present in the plasma are potent modulators of a large number of immune responses, notably inflammation. Such molecules also interfere with coagulation by fostering a pro-coagulant state that, in effect, maintains inflammation (11). Furthermore, inflammatory content of convalescent plasma may potentially interplay with COVID-19 neutralizing antibody transfused with the plasma or pre-existing in the patient (12).
Therefore, as convalescent plasma is being considered to treat COVID-19, careful assessment of inflammatory properties of such plasmas, including the presence of auto-Abs against type I IFNs as described by Bastard et al. (4) is warranted.

Recently Science has published two papers on genetic abnormalities in severe, life-threatening COVID-19 (September 24, 2020). These papers with a great many authors, led by Dr Jean Laurent Casanova, convincingly demonstrate the critical importance of the innate immune mechanisms that regulate the type-1 interferon pathways in COVID-19.
Much to our surprise, both papers completely ignore the paper that we published in JAMA in July, providing evidence that genetic defects in TLR-7 – with strong effects on the downstream type-1 interferon pathway - are associated with lethal COVID-19 in young males (JAMA. 2020 Jul 24;324(7):1-11).
Since the papers in Science were accepted not until September 16, it is unlikely that the authors did not see our paper. It is clear that our paper is in full agreement with the lines of reasoning in the Science papers.
Although I would not like to think that our work was intentionally ignored, I must say that it is amazing that some 100 authors on each paper apparently agreed not to cite it.
