Locker Request
Grades 10 through 12:

To request a locker, please fill out the form below.  All requests are subject to availability.  You may search out a locker of your choice, but it must comply with the following conditions.
1.  Senior lockers must be on the first floor.  
2.  Junior lockers must be on the second floor.
3.  Sophomores must be on the second or third floor.
4.  Freshman lockers are on the third floor.

It is best to locate a locker nearest your homeroom, but can be somewhere else if desired.  If you do not provide a locker of choice, we will assign one to you.
