Zusammenfassung
Nach Schätzungen der WHO enden weltweit 6 von 10 der ungeplanten und 3 von 10 aller Schwangerschaften in einem induzierten Abort (Schwangerschaftsabbruch). Ein im medizinischen Setting durchgeführter Schwangerschaftsabbruch ("safe abortion") hat eine sehr geringe Komplikationsrate. In den meisten Ländern unterliegt der Schwangerschaftsabbruch unterschiedlich starken Restriktionen, die den Zugang zu einem sicheren Abort erschweren oder sogar unmöglich machen. Die Wahl zwischen einem medikamentösen und einem chirurgischen Abort erfolgt nach medizinischen Aspekten unter Beachtung des Wunsches der Schwangeren, ist aber auch durch diverse Reglementierungen mitbeeinflusst. Im ersten Trimester sind medikamentöser und chirurgischer Abbruch als gleichwertig anzusehen, danach erfolgt mit steigendendem Gestationsalter fast nur noch ein medikamentös induzierter Abbruch, um eine Dilatation und Evakuation zu vermeiden. Nach 20 Schwangerschaftswochen wird in der Regel vor Induktion des Abbruchs ein Fetozid durchgeführt. Wichtig ist eine angemessene psychische Betreuung und Unterstützung der betroffenen Eltern, auch bei einem frühen Schwangerschaftsabbruch.
